Membership Change Request

Complete the required form below to submit your membership change request for Platform Strength.

Please also note our policies:

• Notice of intent to hold one’s account MUST be submitted no less than two (2) business days prior to the freeze date, excluding holidays.

• The freeze request must state both the freeze start date and reactivation date.

• Members may put their membership on hold, in one-month increments, for up to three (3) calendar months.

• Cancellation requests must be completed 30 days prior to your membership bill date
